NORTH FLORIDA UNITED BASKETBALL PLAYER LIABILITY WAIVER AND RELEASE FORM

This Liability Waiver and Release Form (“Waiver”) is executed on the day that registration is completed, by the undersigned parent or legal guardian (“Guardian”) on behalf of the minor participant (“Player”), in favor of North Florida Suns, North Florida United, Puma, Puma NXTPRO, Fleming Island High School, First Baptist of Orange Park Church, and any management, staff, volunteers, employees, coaches, sponsors, and representatives affiliated with these organizations (hereinafter referred to as “Released Parties”).

1. Acknowledgment of Risks

I understand that participation in basketball and related activities involves inherent risks, including but not limited to falls, contact with other players, equipment-related injuries, facility conditions, and the potential for serious injury, disability, or death. I voluntarily assume all risks associated with my child’s participation in North Florida United activities.

2. Waiver and Release of Liability

I, on behalf of myself, my child, and our respective heirs, assigns, and representatives, hereby release, discharge, and hold harmless the Released Parties from any and all claims, liabilities, damages, or causes of action, including but not limited to personal injury, death, property damage, or financial loss, arising from or related to my child’s participation in any North Florida United events, practices, games, or activities, regardless of whether such injury or damage was caused by negligence or otherwise.

3. Medical Treatment Authorization

In the event of a medical emergency, I authorize North Florida United and its representatives to obtain emergency medical treatment for my child. I agree that I am financially responsible for any medical treatment, including but not limited to emergency transportation, hospitalization, and medical care.

4. Assumption of Responsibility

I acknowledge that my child is physically and mentally capable of participating in basketball activities and has no known medical conditions that would prevent safe participation. I accept full responsibility for any medical or other expenses incurred as a result of my child’s participation.

5. Indemnification

I agree to indemnify and hold harmless the Released Parties from any claims, demands, lawsuits, or liabilities made by third parties arising out of my child’s actions, participation, or any incident related to North Florida United activities.

6. Consent to Use of Likeness

I grant North Florida United permission to use my child’s name, photograph, and likeness for promotional, advertising, and publicity purposes in any media format without compensation.

7. Governing Law & Severability

This Waiver shall be governed by the laws of the State of Florida. If any provision of this Waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

8. Acknowledgment and Agreement

I HAVE READ AND FULLY UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY. I VOLUNTARILY SIGN THIS DOCUMENT WITH THE INTENT OF RELEASING THE RELEASED PARTIES FROM LIABILITY TO THE MAXIMUM EXTENT PERMITTED BY LAW.
